Health Visitors
Each surgery has a linked Health Visitor. They are specialists in monitoring the health of pre-school children, and supporting families.
Community Midwife
The midwife works with the doctors to provide antenatal and postnatal care.
Community (District) Nurses
The community nurses liaise regularly with the staff and doctors at each surgery. They give advice on illness prevention as well as providing direct care in the community. They can also advise and support carers and friends as necessary and work alongside other agencies.
Community Matron & Case Manager
These are experienced nurses who provide care to elderly patients with longterm conditions and complex needs at risk of hospital admission
Integrated Care Coordinator
The Integrated Care Coordinator is employed by Social Services and provides adirect link for the practice to these services Norfolk Recovery Partnership.
NRP (TADS)
A specialist drug and alcohol worker from NRP provides regular clinics at Gurney Surgery.
Health Trainer
The Health Trainer sees patients each Friday at Gurney Surgery. She provides lifestyle advice and support to patients including help to stop smoking, advice on weight loss, exercise advice and advice on alcohol reduction.
