Castle PartnershipWe are currently in the process of ensuring that we have the most up to date information on your medical record. We have noticed that for some patients we have some areas such as ethnicity or smoking status that are not known.
As such we will be sending a SMS to some patients that will contain a link to a questionnaire. Each link is unique and when you click on this, you will be prompted for your date of birth. This will need to be entered in the DD/MM/YYYY format.
There is only one question, and by answering this it will help ensure that your medical record is updated.
